Кожные инфекции, вызванные бактериями, чувствительными к фузидовой кислоте.
Бактериальные инфекции глаз: конъюнктивит; блефарит; кератит; дакриоцистит.
Септицемия, пневмония, отит, абсцессы, флегмоны, фурункулез, инфицированные ожоги и раны, инфекции мягких тканей, остеомиелит и др., гонорея, вызванная устойчивыми к бензилпенициллину штаммами гонококка, или при повышенной чувствительности больного к пенициллинам.

Таблетки, покрытые оболочкой
Наружно.
Взрослые и дети с рождения до 18 лет. Наносят на пораженные участки кожи тонким слоем 3–4 раза в день.
При использовании марлевых повязок частота нанесения может быть уменьшена до 1–2 раз в день.
Продолжительность лечения зависит от формы и тяжести заболевания и в среднем составляет 7–14 дней.
Конъюнктивально. По 1 капле в конъюнктивальную полость пораженного глаза 2 раза в день в течение 7 дней.
В случае отсутствия положительной динамики в течение 7 дней применения препарата следует пересмотреть терапию, используя другие ЛС.
Внутрь, после еды. Взрослым разовая доза составляет обычно — 0,5 г, суточная — 1,5 г в 3 приема с интервалом 8 ч. Средняя продолжительность курса — 7–14 дней, при остеомиелите — 2–3 нед. Детям назначают из расчета 40–60 мг/кг, а при тяжелом течении болезни — до 80 мг/кг /сут.

Ниже приводятся данные о неблагоприятных побочных реакциях, в зависимости от частоты из возникновения: очень часто — ≥1/10; часто — ≥1/100, <1/10; нечасто — ≥1/1000, <1/100; редко — ≥1/10000, <1/1000; очень редко — <1/10000.
Со стороны кожи и подкожных тканей: нечасто — сыпь (эритематозная, макулопапулезная, пустулезная), раздражение кожи, включая боль, чувство жжения, чувство покалывания на коже, зуд, сухость кожи, контактный дерматит; редко — крапивница, ангионевротический отек, экзема, периорбитальный отек, эритема.
Со стороны иммунной системы: редко — аллергические реакции.
Со стороны органа зрения: редко — раздражение конъюнктивы.
Если любые из указанных в инструкции побочных эффектов усугубляются, или пациент заметил любые другие побочные эффекты, не указанные в инструкции, следует сообщить об этом врачу.
Редко — аллергические реакции в виде отека фолликулов переходной складки конъюнктивы и крапивницы. Возможны — зуд, жжение, конъюнктивальная гиперемия, болезненность, слезотечение, отек век, обострение конъюнктивита.
Терапия Фуциталмиком должна быть прекращена в случае появления признаков гиперчувствительности к препарату.
Боль в животе, тошнота, рвота, понос, обусловленные местным раздражающим действием препарата на слизистую оболочку ЖКТ; аллергические явления (покраснение слизистой оболочки ротовой полости и глотки, боль при глотании, кожные сыпи).

Бактериостатический антибиотик.
Фузидовая кислота относится к группе фузидинов, антимикробных соединений, механизм действия которых связан с нарушением синтеза белка в бактериальной клетке.
К фузидовой кислоте чувствительны грамположительные бактерии и грамотрицательные кокки, например Staphylococcus aureus, Staphylococcus epidermidis (включая метициллинрезистентные штаммы), Nocardia asteroides, Corynebacterium minutissimum, Streptococcus pyogenes, Pseudomonas spp., Clostridium spp., Neisseria meningitidis, Neisseria gonorrhoeae.
Неактивен в отношении Escherichia coli, Salmonella spp., Proteus spp. и других грамотрицательных бактерий, а также простейших и грибов.
Фузидиевая кислота относится к группе фузидинов, антимикробных соединений, механизм действия которых связан с нарушением синтеза белка в бактериальной клетке. Блокируя элонгацию фактора G, они предотвращают его связывание с рибосомами и гуанозинтрифосфатом, что прерывает высвобождение энергии, необходимой для синтеза белка и приводит к гибели бактериальной клетки.
К фузидиевой кислоте чувствительны следующие микроорганизмы, наиболее часто вызывающие глазные инфекции: Staphylococcus aureus, Streptococcus pneumoniae, Haemophilus influenzae.
Enterobacteriaceae и Pseudomonas устойчивы к действию фузидиевой кислоты.

Проницаемость фузидовой кислоты через интактную кожу низкая. В системный кровоток не попадает.
Лекарственная форма Фуциталмика в виде вязких глазных капель обеспечивает длительный контакт препарата с конъюнктивой и достаточную концентрацию фузидиевой кислоты в слезной жидкости при условии применения 2 раза в сутки. Через 1, 3, 6 и 12 ч после применения однократной дозы Фуциталмика средняя концентрация фузидиевой кислоты в слезной жидкости составляет 15,7; 15,2; 10,5 и 5,6 мкг/мл соответственно. Во внутриглазной жидкости концентрация фузидиевой кислоты, равная 0,3 мкг/мл (после однократного применения) и 0,8 мкг/мл (после повторного применения), достигается в течение 1 ч после применения и поддерживается, по крайней мере, в течение 12 ч, при этом фузидиевая кислота в сыворотке крови не определяется.
При длительном применении может наблюдаться кумуляция. При введении внутрь хорошо всасывается и достигает в крови высоких концентраций. Проникает в кожу, подкожную клетчатку, легкие, печеночную ткань, кости, плевральный экссудат, хрящевую и соединительные ткани. Из организма выводится, в основном, с желчью и, в меньшей степени, с мочой.

Treatment of non-severe, superficial, non-extensive, primary skin infections caused by microorganisms that are sensitive to Fusidate De Sodium Pierre Fabre 2%, especially of infections caused by Staphylococcus.

Bacterial resistance has been reported to occur with the use of Fusidate De Sodium Pierre Fabre 2%.
Extended or recurrent use may increase the risk of developing contact sensitisation.
When Fusidate De Sodium Pierre Fabre 2% 20 mg/g cream is used on the face, care should be taken to avoid the eyes, because Fusidate De Sodium Pierre Fabre 2% can cause irritation of the conjunctiva.
Fusidate De Sodium Pierre Fabre 2% 20 mg/g cream contains butylhydroxyanisole, cetyl alcohol and potassium sorbate which may cause local skin reactions (e.g. contact dermatitis). Butylhydroxyanisole may also cause irritation to the eyes and mucous membranes.
Bacterial resistance among staphylococcus aureus has been reported to occur with the use of topical Fusidate De Sodium Pierre Fabre 2%. As with all antibiotics, extended or recurrent use may increase the risk of developing antibiotic resistance.
Extended or recurrent use may increase the risk of developing contact sensitisation.
Fusidate De Sodium Pierre Fabre 2% cream contains butylhydroxyanisole, cetyl alcohol and potassium sorbate. These excipients may cause local skin reactions (e.g. contact dermatitis). Butylhydroxyanisole may also cause irritation to the eyes and mucous membranes. Fusidate De Sodium Pierre Fabre 2% cream should therefore be used with care when applied in the proximity of the eyes.

Active mechanism:
Fusidate De Sodium Pierre Fabre 2% belongs to a unique group of antibiotics, the fusidanes, which act to inhibit bacterial protein synthesis by blocking the lengthening of factor G. This is to prevent it from associating with ribosomes and GTP, thus preventing energy supply to the synthesis process.
As it is the only type of drug available in this family of drugs, there have been no reports of cross resistance to Fusidate De Sodium Pierre Fabre 2%.
Resistance mechanism(s):
Resistance for Fusidate De Sodium Pierre Fabre 2% can vary geographically and information about local resistance patterns should be obtained through a local microbiology laboratory. In general, resistance occurs in 1-10 % of Staphylococcus aureus and 10-20 % of coagulase negative staphylococcus. Cross-resistance between Fusidate De Sodium Pierre Fabre 2% hydrophilic cream 20 mg/g and other antibiotics has not been reported.

Фармакологическое действие
Антибиотик. Оказывает бактериостатическое или бактерицидное действие. Механизм действия связан с подавлением синтеза белка за счет ингибирования фактора, необходимого для транслокации белковых субъединиц и элонгации пептидной цепи, что приводит в дальнейшем к гибели возбудителя.
Высокоактивен в отношении Staphylococcus spp., особенно Staphylococcus aureus и Staphylococcus epidermidis (включая метициллин-резистентные штаммы), Nocardia asteroides, Clostridium spp.
Менее активен в отношении Streptococcus spp., Enterococcus spp., Neisseria spp., Bacteroides spp., Mycobacterium tuberculosis, Mycobacterium leprae.

Режим дозирования
Взрослым внутрь — по 500 мг каждые 8 ч. Детям в возрасте до 1 г — 50 мг/кг/сут в 3 приема, в возрасте 1-5 лет — по 250 мг 3 раза/сут, в возрасте 5-12 лет — по 500 мг 3 раза/сут, старше 12 лет — по 750 мг 3 раза/сут.
Взрослым и детям старше 12 лет внутрь — по 250-500 мг 2-3 раза/сут. Взрослым в/в в виде инфузий — по 500 мг 3 раза/сут. Длительность инфузии — не менее 2 ч.
Детям в возрасте до 12 лет внутрь — 50 мг/кг/сут в 3 приема; в/в — 20 мг/кг/сут, кратность введения — 3 раза/сут.
Максимальная суточная доза для взрослых при в/в введении составляет 2 г.
При наружном применении фузидовую кислоту наносят тонким слоем на пораженные участки кожи 2-3 раза/сут в течение 7 дней. При лечении обыкновенных угрей требуются более длительные курсы терапии.
Местно при заболеваниях глаз применяют 2 раза/сут. Лечение следует продолжать в течение 2 дней после исчезновения клинических симптомов заболевания.
Побочное действие
Со стороны пищеварительной системы: возможны диспептические симптомы, желтуха, нарушение функции печени.
Аллергические реакции: возможна кожная сыпь.
Противопоказания
Повышенная чувствительность к фузидовой кислоте.
Беременность и лактация
При необходимости применения при беременности и в период лактации следует оценить предполагаемую пользу для матери и потенциальный риск для плода.
B экспериментальных исследованиях установлено, что фузидовая кислота не оказывает тератогенного действия.
Фузидовая кислота проникает через плацентарный барьер. В экспериментах in vitro установлено, что фузидовая кислота замещает билирубин в связи с альбумином. Поэтому следует избегать применения фузидовой кислоты в III триместре беременности из-за возможного развития ядерной желтухи у новорожденных. Этот факт следует учитывать при необходимости применения у новорожденных (в т.ч. недоношенных) детей с желтухой и ацидозом.
Фузидовая кислота выделяется с грудным молоком в очень низких концентрациях.
Применение при нарушениях функции почек
Следует учитывать, что фузидовую кислоту можно применять при нарушениях функции почек и в случаях, когда противопоказано применение других антибиотиков.

Фармакологическое действие
Бактериостатический антибиотик. Относится к группе фузидинов, противомикробным соединениям, механизм действия которых связан с нарушением синтеза белка в бактериальной клетке.
К фузидовой кислоте чувствительны грамположительные бактерии и грамотрицательные кокки, например, Staphylococcus aureus, Staphylococcus epidemridis (включая метициллин- резистентные штаммы), Nocardia asteroides, Corynebacterium minutissimum, Streptococcus pyogenes, Pseudomonas spp., Clostridium spp., Neisseria meningitidis, Neisseria gonorrhoeae.
Не активен в отношении Escherichia coli, Salmonella spp., Proteus spp., а также простейших и грибов.
Фармакокинетика
При наружном применении системная абсорбция фузидовой кислоты низкая. В системный кровоток не попадает.
Дозировка
Крем и мазь применяют наружно.
У взрослых и детей с рождения до 18 лет крем или мазь наносят на пораженные участки кожи тонким слоем 3-4 раза/сут.
При использовании марлевых повязок частота нанесения может быть уменьшена до 1-2 раз/сут.
Продолжительность лечения зависит от формы и тяжести заболевания и в среднем составляет 7-14 дней.
Передозировка
Передозировке препарата Фуцидин® маловероятна.
Лекарственное взаимодействие
Лекарственное взаимодействие препарата Фуцидин® не выявлено.
Применение при беременности и лактации
С осторожностью назначают препарат при беременности и в период лактации (грудного вскармливания).
B экспериментальных исследованиях установлено, что фузидовая кислота не оказывает эмбриотоксического и тератогенного действия.
Побочные действия
Побочные эффекты наблюдаются очень редко и могут ожидаться примерно у 5% пациентов.
Определение частоты побочных реакций: очень часто (> 1/10), часто (> 1/100 и < 1/10), нечасто (> 1/1000 и < 1/100), редко (> 1/10 000 и < 1/1000), очень редко (<1/10 000).
Дерматологические реакции: нечасто — сыпь (эритематозная, макулопапулезная, пустулезная), раздражение кожи, включая боль, чувство жжения, чувство покалывания на коже, зуд, сухость кожи, контактный дерматит; редко — экзема, периорбитальный отек, эритема.
Аллергические реакции: редко — крапивница, ангионевротический отек, экзема.
Со стороны органа зрения: редко — раздражение конъюнктивы.
Пациент должен сообщить врачу о возникновении любых указанных или не указанных выше побочных эффектах.
Условия и сроки хранения
Крем следует хранить при температуре не выше 25°C. Срок годности — 3 года.
Мазь следует хранить при температуре не выше 30°C. Срок годности — 3 года.
Препарат следует хранить в недоступном для детей.
Показания
— кожные инфекции, вызванные бактериями, чувствительными к фузидовой кислоте.
Противопоказания
— повышенная чувствительность к компонентам препарата.
Особые указания
Не наносить Фуцидин® на кожу в области глаз, поскольку фузидовая кислота, при попадании в глаза, вызывает раздражение конъюнктивы.
Крем содержит бутилгидроксианизол, цетиловый спирт и калия сорбат, а мазь — ланолин и цетиловый спирт. Эти вспомогательные вещества могут быть причиной появления местных аллергических кожных реакций (например, контактного дерматита).
Бутилгидроксианизол также может послужить причиной появления раздражения глаз и слизистых оболочек.
При применении фузидовой кислоты возможно развитие бактериальной устойчивости. Как и у других антибиотиков, длительное или периодически повторяющееся использование может увеличить риск развития устойчивости к антибиотикам.
Влияние на способность к вождению автотранспорта и управлению механизмами
Влияние препарата на способность управлять транспортными средствами и сложными механизмами не выявлено.
